Patient Information
Here's what you should bring to your appointment:
- Your insurance card
- A State Issued photo ID
- List of prescriptions including over the counter medication.
- Information about your medical and surgical history.
- Recent x-rays or relevant records.
- Copay, co-insurance, and any deductible due.
Please note: If for any reason you are unable to keep your appointment, it is very important that you contact our office to cancel. We do not overbook our appointments, we reserve that time for you and we have other patients who want to get in as soon as possible that we can give that appointment to. Failure to keep your appointments may result in termination from our practice.

Insurance Information
Tennessee Valley Urology Center, P.C. is a participating provider for Medicare and most major insurance plans, however due to the complexity of the insurance company networks, it is vital that you check with your insurance plan to see how your visit will be covered.
As a courtesy to our patients we are pleased to file claims to most insurance companies. However, we will collect any co-pays, co-insurance, and deductible amounts at the time of serivce. We accept cash, check, Visa, and Mastercard.
